Automated phone calls and text messages are sent to parents or guardians daily notifying them when a student has been marked absent.  In the text or email the guardians will be provided a link to submit the parent note explaining the reason for the absence.  It is the parent's or guardian's responsibility to ensure that the most current and up-to-date contact information is on file at their child's school. Parents or guardians must in some cases provide documentation explaining why the student was absent. NOT all reasons are excused.

Note: The school must receive this completed form and any required documentation within 3 school days of your child's return to school.
